After release, the daily atazanavir dose was changed to 300 mg boosted by 100 mg of ritonavir. Treatment with 8 mg of buprenorphine was also initiated at a dose of 8 mg per day. However, the patient complained of dizziness and feeling “high” and the dose of buprenorphine was reduced to 8 mg every other day. Following symptoms of opiate withdrawal, the buprenorphine dose was subsequently increased to 12 mg per day and this caused sleepiness similar to that observed in the other 2 patients.
