Добрый день. Помогите, пожалуйста, с переводом Fax Schedule Set-Up Form в данном контексте:

The Customer shall fax a Fax Instruction in respect of the Account(s) specified in the Fax Schedule Set-Up Form only to the Bank's fax number specified in the Fax Schedule Set-Up Form or to another fax number the Bank may otherwise specify in writing from time to time. Should the Bank receive a Fax Instruction sent to a number either not listed in the Fax Schedule Set-Up Form for that purpose or otherwise not specified by the Bank, the Bank may disregard it. The Customer must notify the Bank in writing of any changes to the Fax Schedule Set-Up Form. Changes are valid three Business Days after receipt of such written notice.
