речь, по сути, о передаче авторских прав, но это не договор, а как бы заявление. начинается так:
For good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, I, ----, hereby certify that, at the request of ----- ("Producer"), I have rendered and will continue to render services...
