Release agreement

I hereby acknowledge that I have been requested by UEFA to act as an [insert] in connection with the final tournament of the UEFA European Football Championship 2010/2012 (“UEFA EURO 2012”) and agree to my official performance and/or appearance at events in connection with UEFA EURO 2012 being filmed and recorded in any and all media of whatever nature. I further consent and hereby grant to UEFA all permissions, consents and waivers necessary to enable UEFA to use, exploit, license and/or sub-license my name, nickname, likeness, performance, appearance and/or voice as a part of any such audio, visual, audio-visual and/or electronic recordings of UEFA EURO 2012 in any and all current and/or future media throughout the world in perpetuity (including the waiver of any and all image, personality, appearance or similar rights under any applicable laws). I shall not assert any claim to use, sell or exploit any of the rights granted to UEFA pursuant to this release agreement in connection with UEFA EURO 2012 or otherwise.
I further acknowledge that I have read, understood and agree to be bound by the terms and conditions of this release agreement.
