print name - расшифровка подписи type name - имя параллельное подписание/оформление/исполнение контракта, как в следующей ситуации: This Agreement may be executed by fascimile and in counterpart ... provided that either Party executing by facsimile shall promptly provide the other Party with an original of its signed execution page of this Agreement. A valid and binding contract shall arise if and when counterpart execution pages (including as may be delivered by fascimile) are executed and delivered by each Party to the other Party. |

print or type name - печатными буквами. Это где-то рядом с подписями? Если я правильно понял, требуется указать имя ПЕЧАТНЫМИ БУКВАМИ. |
