Мой вариант - "appearance at work under the influence of alcohol, drugs or controlled substances". (хоть мне и не нравится выражение appearance at work)

По-моему, "appearance at work" нормально, а вот "controlled substances" вызывает сомнение. Согласен с Мо (or other intoxication).

consider: присутствие на работе = attendance.

Возможно вы и правы насчет controlled substances. Это весьма распространенная формулировка и здесь ИМХО чисто юридические фишки, чем просто перевод. Кленя тоже можно нанюхаться и стать intoxicated, но клей не относится к CS.

... reporting for work while under the influence of alcohol, illegal drugs, or other substances.... (if you need it word-for-word)

As I recall in a couple of my employment contracts, they called it simply "reporting to work intoxicated" and left out intoxicated with what.
