в MT приведены два очень разных перевода personal background - автобиография и личная квалификация. По контексту в принципе уместны оба варианта (перечень документов кандидата), но они ведь очень разные по сути.
Вот хочу спросить общественность на этот счет.

Персональная информация, личные сведения или типа того...
контекст все же нужен, вы правы
Applications for a licence must be made on forms provided
