В заявлении в обязательном порядке должны быть заполнены все графы, за исключением граф «Рекомендации комиссии», в графе «Примечание» допускается проставление прочерка.

мой вариант:

All the columns should obligatory be filled-in in the application , except for the column “Recommendations of the Committee”, dash is allowed in column “Note”.

Англоговорящие не ставят прочерк, а пишут в таких случаях N/A.
