Вот образец, так же переведенный когда-то с русского (как вариант):

I, Notary Public of ....(или practicing in.....), acting under/upon license....(если дальше идет номер, то "лицензия" без артикля), do hereby certify that this is a true and complete copy of the original document. In the latter no erasures, interlineations, crossed out words and other unauthorised corrections or peculiarities have been found.
