In the CAPRIE study, both clopidogrel therapy and medium-dose aspirin therapy were well tolerated. The overall incidence of hemorrhagic events was identical in the aspirin and clopidogrel groups (9.3%) (CAPRIE Steering Committee 1996). In the CURE trial, there were significantly more patients with major bleeding in the clopidogrel group than in the placebo group (3.7% vs 2.7%, respectively; p = 0.001) (Yusuf et al 2001). Interestingly, a subgroup analysis of the CURE trial73 based on the daily dose of prescribed aspirin revealed a clear dose-related effect on major bleeding, in both treatment groups. Those patients receiving the lowest dose of aspirin (ie, ≤ 100 mg) combined with clopidogrel had virtually the same rate of major bleeding complications (3.0%) as those receiving higher doses of aspirin and placebo.
