Create a referral system. In addition to the Case Management Teams' service provision responsibilities, in the second month of project implementation, the Teams will create a system of inter-organizational cooperation for the purpose of referral. This will outline referral mechanisms and relationships between NGO activities and state-provided mother and child heath services, and will result in detailed Memoranda of Understanding among DOW, AUN, partner NGOs, and state health care facilities. The AIDS Center already hosts activities targeting HIV affected children. The Director has endorsed the proposed project and expressed his readiness to facilitate Case Management Teams access to pregnant and birthing women, making the AIDS Center a member of this inter-organizational referral system.

"Система отсылок"? "Система направления в соответствующие учреждения за помощью"? А может, все же, это система справочной информации, которой будут обладать организации, охваченные проектом как участники и партнеры?

скорее, система направления.
когда терапевт посылает еще к кардиологу, это называется referral.
по-русски это "направление"

Вроде еще шире, чем "Система направления в соответствующие учреждения за помощью", как я погляжу дальше:

Referrals will include facilitated access to: state-provided medical services; the relevant social protection agency, which provides social assistance; other AUN programs; the Project Legal Advisors, and projects run by other international and national NGOs. This may entail assitance with completion of documentation, formally complaining about discrimination, liaising with state institutions, and educating the mother and family members regarding available services.
