about a mile over the hill - IMHO, означает, что он еще не старый, но уже близок к тому.

'over the hill' is a colloquial idiom meaning 'old'. The idea is that he's gone over the top of the hill (the best years of his life) and is now sliding downwards towards retirement and death.

The author's reinforced this by saying that he's not just 'over the hill' but a mile over it -- so 'very old'.

I think so, yes -- large and strong, but possibly too old to be doing body-building, which could explain why he's sweating a lot.

Slightly more polite/euphemistic:

* past (his/her) prime #

More blunt:
* an old fogey
* an old fart
* an old geezer

* an old dearie [women only]
* an old bat [women only] [especially if she's annoying/difficult to live with]
