Я бы заменил concluded на made, а в скобочках написал hereinafter referred to as the "Bank"

В скобочках можно просто написать the Bank (без кавычек и без hereinafter referred to as)
ну и dated лучше написать без as of , т.е. как у самого Ko4evnik'a

Я в свое время много переводила канадских контарктов по кредитам. Там всегда было dated as of . На своем мнении не настаиваю.

А у меня в британских встречается, просто, dated (вот ведь, блин, по-разному все пишут)
P.S. Канадского ни разу, кстати, ничего не переводил. А, не, вру! Один раз был устав "монреальский" . Но там с dated ничего не встречалось...
