1) УСИЛЕНИЕ. Эмфатицкая конструкция, часто с запятой после never. 2) afraid to do sth по моим набл. несколько отличается от afraid of doing smth. Думаю, Мэрфи об этом говорит. А насчет третьей конструкции - примерчик из BBI COmbinatory dictionary of English afraid adj. 1. deathly, terribly afraid 2. afraid of (the child was afraid of the dark) 3. afraid to + inf.
