Пожалуйста, помогите перевести нотариальное заверение документа. Наверное, есть какие-то клише в переводе, но я их не знаю.

я, частный нотариус городского нотариального округа,
удостоверяю подлинность подписи Иванова О.Б., который проживает в городе N, который лично обратился ко мне, и известный мне как лицо, указанное в этом документе, подписал его в моем присутствии и надлежащим образом подтвердил оформление этого документа.
