•Supervisors, First Aider and injured person will asses condition and determine the extent of the injury and whether professional medical care is needed. If evaluation concludes that professional medical care is not needed then together the supervisor, first aider, and injured person will decide on appropriate duties – normal or modified.
•If the supervisor, first aider and injured person decide that professional medical care is needed then the necessary documents including the modified duties package will be provided to the injured person prior to leaving the site for completion by the medical professional.
Modified Duties Package shall include:
• Document describing possible modified duties available.
• Modified Duty document (see example in Annex 1) to be completed by medical professional
