i definitely do not think it's a fake tongue piercing. this isn't, however, a common saying. it is something the band has come up with and, perhaps, the meaning is in some other song lyrics of the same band. (bands often get self-referential). however, after consulting with friends, we feel it is probably something along the lines of 'cocksucker', someone who tongues (licks) balls (testicles). thematically it fits as it is used in the pejorative. if you found something else, i'd be curious to find out what it is.:))
