In the UK, I've no doubt the receptionist would write down the time and date of your appointment if you asked her to -- for example, because you had a poor memory -- and some surgeries may have a printed slip designed for this purpose, but it's not something you need in order to see the GP, and most people won't have heard of it. If you have an appointment your name's simply called out while you're sitting in the waiting room, and if you don't have one it isn't, so you don't get to see a GP. If you're due to see a specialist in a hospital, they'll probably send you a letter with details of your appointment date and time beforehand, but you don't need it in order to see him/her. If you want time off work because you're ill, you get a 'sick note' from your GP, which states that you need x days or x weeks off work in order to recover. If your translation's for a UK customer, you could always put something like 'a note/letter from your surgery/hospital confirming that you have a doctor's appointment'. If it's for a non-UK customer, you'll need to put something different as they'll have very different systems, of course.
